Fluttering of the mitral valve in aortic insufficiency.

Thirty-five patients with clinical evidence of aortic insufficiency were studied by echocardiography. Eleven patients had characteristic diastolic fluttering of the mitral valve (30 to 40 Hz, maximal amplitude 4 mm). In five others the fluttering was classified as equivocal. In a control group of 500 echocardiograms in patients without evidence of aortic insufficiency, there were three examples of equivocal fluttering. Atrial fibrillation also produces diastolic fluttering but of slower frequency.

We present a child with truncus arteriosus and truncal insufficiency who had diastolic fluttering of the anterior mitral valve leaflet and left ventricular surface of the interventricular septum on echocardiogram. Although the fluttering has been previously described in patients with aortic insufficiency, it has not been reported with truncus arteriosus.

Echocardiography of the "floppy" aortic valve. Report of a case.

The echocardiographic features of a patient with severe aortic regurgitation due to a "floppy" aortic valve are presented. The salient abnormality observed was marked fluttering of the aortic valve cusps in diastole. Fluttering of the anterior leaflet of the mitral valve and left ventricular enlargement were noted. The diagnosis of a "floppy" aortic valve was substantiated at surgery.
